How to find the Medical Assessment Form link

To access the Initial Voucher/Assessment form link, follow these steps:

1. Click on the User icon located at the top right corner of the page.
2. Select “Dashboard” from the drop-down menu.
3. In the left menu column, click on “My Subscription/subscriptions.”
4. If you have only one subscription, it will display your single subscription details. Otherwise, you’ll see a list of your subscriptions.
5. Choose the subscription for which you need to fill out the assessment form.
6. Scroll down and you’ll see a table of all your available assessment forms and their status

Note: Not all products require an assessment form to be filled.
